Directory Information Withhold Request - Parents Or Guardians may print and fill out the directory information withhold request (to restrict 3rd party mailings) and must be submitted to your school's administration office annually.

FERPA Annual Notice And Directory Information Notice

  • Annual Notice - Administrators at each school are responsible for the distribution of the annual FERPA notice and directory information notice by publishing the required policy information in school handbooks annually. See page 5 of the Administrative Memo (section on FERPA and Directory Information) [Link to PDF in District Policy Manual]

  • School Handbooks - The directory information notice is included annually in student handbook publications from each school. The Administrative Memo 28 above indicates district policies to include in the handbook publication -- including the FERPA directory information notice.

GDPA - Government Data Protection Act implemented to protect all personal data in Utah government entities, which will extend data protections and procedures to all personal data collected by Davis School District (students, parents, employees, and any other). Documentation here will include

Student Data Protection Act (SDPA) -- Act under which the district is required to demonstrate compliance to privacy practices, including the notifications, metadata dictionary (list of products used) and requrements to have clear data privacy agreements with 3rd-party vendors.
